Intake Forms

• Please be sure to complete the intake/ consent forms prior to your visit, so we can review everything in detail before your appointment.

• You can upload a copy of your completed forms and any recent lab work or imaging reports to your secure patient portal found here. Alternatively, you can fax them to us or drop them off at the clinic.

• The intake forms take about 30minutes to complete.

• Please also send us or bring in any recent lab work or imaging, if you have copies of it.
