We’re excited to meet you! To help us prepare for your visit, please complete the intake/consent forms before your appointment. Here’s what you need to know:

• Forms: After you book your initial consultation, the forms will all be emailed to you. You can fill them out on a computer, phone, or tablet before your appointment.

• Need Help?: If you have any trouble completing the forms, feel free to contact us. We're always happy to assist!

• Review Process: Dr. Vosburgh, ND, typically reviews the intake forms about a week before your appointment. The sooner you submit them, the more time she has to prepare.

• Time Estimate: The forms take about 30 minutes to complete.

• Additional Documents: If you have any recent lab work or imaging, please send us copies or bring them with you.

• Pediatric Initial Consultation- 12 and under (60 min): $185

• Follow-up (60 min): $185

• Follow-up (45 min): $140

• Follow-up (30 min): $95

• Acupuncture Initial Consultation (60 min): $150

• Acupuncture Follow-up (30 min): $75

• I.V. sitting fee: $90 (plus cost of IV ingredients)

Please note our strictly enforced 24 hour cancellation policy (50% of the visit fee is charged if less than 24 hours notice is provided). Please notify us at least 24 business hours before your appointment to avoid penalty to help us continue to provide the best quality of care and availability to all patients.
